Filing A Claim

In the event of a loss, call Hayes, Utley & Hedgspeth at

Be prepared to provide as much of the following information as possible:

• Your policy number

• Details about the loss, and how it occurred, including time, date and place

• The file number for the accident or loss report, if one was filed by local law enforcement.

• Contact information on where to reach you for the next 48 hours.

If it is outside of office hours and you cannot reach us directly for claim emergencies, such as a fire or injury causing accident, notify your insurance company direct at the one of the following numbers.

In the event of an injury, most insurance companies prefer you contact them direct using their toll-free phone numbers, if the claim is not filed by the health care provider

Consult your policy to confirm the name of your insurance company, then call the appropriate number below.
